Explosion and Fire at Williams Olefins Plant, Geismar, Louisiana 

Blowout in OK

A training video for the onshore drilling industry detailing lessons learned from the January 22, 2018, blowout and fire at the Pryor Trust gas well in Pittsburg County, Oklahoma, that fatally injured five workers. 

Death in the Oilfield

On June 5, 2006 there was a fatal explosion at a rural oil production field in Raleigh Mississippi.  Three contractors died and one contractor suffered serious injuries in an explosion and fire at the Partridge-Raleigh Oilfield. The contractors, all employees of Stringer Oilfield Services, were standing on top of a series of four oil production tanks. They were preparing to weld piping to the tanks when a welding tool likely ignited flammable vapors from the tanks.

Filling Blind

CSB Safety Video on the 2009 massive explosion at the Caribbean Petroleum, or CAPECO, terminal facility near San Juan, Puerto Rico. The incident occurred when gasoline overflowed and sprayed out from a large aboveground storage tank, forming a 107-acre vapor cloud that ignited. While there were no fatalities, the explosion damaged approximately 300 nearby homes and businesses and petroleum leaked into the surrounding soil, waterways and wetlands. 

Fire From Ice

On February 16, 2007, a propane fire erupted at the Valero McKee Refinery in Sunray, Texas, north of Amarillo. Three workers suffered serious burns, and the refinery was forced to shut down. The fire began following a leak in the propane deasphalting unit and spread quickly, in part because of the rapid collapse of a major pipe rack carrying flammable hydrocarbons. Some of the rack's support columns had not been fireproofed. 

H2S Fatality

A CSB safety video on the October 26, 2019, hydrogen sulfide release at the Aghorn Operating waterflood station in Odessa, Texas. The release fatally injured an Aghorn employee who was working at the facility that evening, as well as his spouse who attempted to locate him at the facility after he did not return home.

No Escape

On October 2, 2007, five people were killed and three others injured when a fire erupted 1,000 feet underground in a tunnel at Xcel Energy Company's hydroelectric power plant in Georgetown, Colorado, located approximately 45 miles west of Denver. This CSB safety video explains how the accident occurred.

Refinery Fire Baton Rouge

On November 22, 2016, a fire erupted during maintenance activities at the ExxonMobil refinery in Baton Rouge, LA, severely burning four workers. 

Refinery Fire Philadelphia

A CSB safety video about the June 21, 2019, fire, explosions, and toxic hydrofluoric acid (HF) release at the Philadelphia Energy Solutions refinery in Philadelphia, Pennsylvania. A severely corroded pipe elbow ruptured, releasing process fluid into the refinery’s HF alkylation unit, which then ignited. The incident caused the release of over 5,000 pounds of highly toxic HF, launched a 38,000-pound vessel fragment off-site, and resulted in an estimated property damage loss of 750 million dollars.
